Each year, the Foundation organizes the annual Associate Campaign as an opportunity for our community to come together in support of our Mission. Last year, associates across the state contributed more than $450,000 to support important programs in local Ascension St. Vincent ministries. 

Your donation will directly impact the fund you choose. A list of priority programs and services for local ministries across the state is available here. Thank you to our associates who have already pledged their support!

Please complete the payroll deduction form below or on myAscension by clicking My Links > Charitable Contributions. Payroll deduction gifts automatically recur bi-weekly with each pay period unless the Foundation is notified otherwise.

•  Your Department ID is listed on your pay documents. 
•  Deductions submitted on the form below will begin in January 2023 with the first paycheck. Payroll deductions made after January 1 will be deducted from the next payroll after form is submitted. 
•  Your Associate ID is the first 8 digits of the 10-digit number on the back of your badge.
