Ascension St. Vincent Foundation and Health Care Center, Inc. (''St. Vincent'') is committed to the privacy and confidentiality of personal information. In order to provide you with accurate, useful and customized information, Ascension St. Vincent asks for the minimally necessary data which may include your name, address, email and relevant health history along with information about lifestyle behaviors such as smoking and exercise, physical measurements such as height weight and blood pressure, and mental health topics. Your answers form the basis of the personal information that is unique to you. In conjunction with the customized information provided, you may also receive follow-up mailings.

Any transactions performed requiring your credit card information are performed in a secure environment, and credit card information is discarded immediately upon completing the necessary transaction(s). 

Within the web site, you may be given the opportunity to indicate your preference to receive program information or participate in future research studies. If you indicate a preference to receive information, Ascension St. Vincent will use your contact information to send you information about Ascension St. Vincent, material about related products, or invitations to participate in research studies to assess the effectiveness of Ascension St. Vincent's programs.

Ascension St. Vincent is committed to protecting the privacy of children. ASCENSION ST. VINCENT'S PRODUCTS ARE NOT INTENDED FOR USE BY ANYONE UNDER THE AGE OF 18. In the event the system recognizes that anyone under 18 has attempted to send personal information through questionnaires, the session will be aborted and an appropriate notice posted to the user.
