Thank you for supporting Health Access at Ascension St. Vincent!

Health Access at Ascension St. Vincent is a community-based care coordination program focused on improving access and removing barriers to needed resources to better connect our friends, family and neighbors to health, human and social services. Whether an individual is a patient of Ascension St. Vincent or not, Health Access is here to help!

There are 16 Health Access offices across Indiana. Types of services provided include:
  • Enrollment: Includes Marketplace, Indiana Coverage Programs, WIC, Social Security, etc.; 1,756 people were helped last year
  • Social Service: Includes housing, utility assistance, financial assistance, transportation, DME, employment, Education, etc; 11,999 people were helped last year
  • Pregnancy: Includes assisting in the pregnancy journey to have a successful pregnancy; 139 people were helped last year
  • Medical Home: Includes establishing with a Primary Care Physician; 471 people were helped last year
  • Food Insecurity: Includes assistance in applying for SNAP, providing food pantry options, gift cards, etc

Current greatest needs to support the work of Health Access include non-perishable items and personal care products to be available at food pantry locations across Indiana, as well as transportation and safety items.
